The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at an undisclosed speed, there was a loud sound coming from the engine with the message "Service Hybrid System" displayed. The sound was noticeable while accelerating and decelerating. The vehicle was taken to the dealer, but no DTC was retrieved; however, the contact was informed that the transmission control module needed to be reprogrammed. The TCM was reprogrammed; however, the issue persisted with a high-pitched whining sound. The manufacturer was notified of the issue. The approximate failure mileage was 48,000.
ELECTRICAL SYSTEMApril 5, 2026
While driving on the highway, the vehicle unexpectedly entered “limp mode”. Approximately 10 seconds before the vehicle reduced power, a warning message appeared indicating the vehicle was about to shut down. This created a dangerous situation because we were traveling in the middle lane of a highway with vehicles moving at significantly higher speeds on both sides. We were able to carefully move to the side of the road and eventually exit the highway; however, the vehicle was limited to approximately 15–20 mph and could not safely travel at normal speeds due to limp mode. There is an existing recall related to this issue. The vehicle had already received the software update that provides advance warning before shutdown. While the warning is helpful, it does not address the underlying safety issue, which appears to be related to a hardware problem involving faulty transmission wiring rather than software alone.

At Random, the dashboard will light up with a red brake light, the check engine light, front end collision warning, and the ABS warning light. IT gives me several messages as well. Service electronic stability control, Auto emergency Braking Active Braking off, Auto emergency Braking unavailable Service Required. When this happens the brake pedal is impossible to push and it takes every ounce of power to get the vehicle to stop. This is no warning that this is coming and it happens most often when you are driving. This puts myself and my family at risk as well as everyone that is driving near me when this happens. It has been repaired 6 times by 2 different dealerships, and it still isn't fixed.

The vehicle completely stopped and shut off while driving on the roadway. Luckily no one was behind me. The check engine light did come on right before but it wasn’t on long enough for me to get somewhere to stop and check it out. There had not been any issues prior to this. The vehicle is still being serviced but they said it was the 12v battery and electric coolant heater that needs replacing so far.

My vehicle has been saying engine will shut off soon, and loses power . Transmission is the issue as I have had it diagnosed 3 times with the code P0a43. Mazda dealership which I purchased the vehicle verified by running a diagnostic.engine light comes on after the vehicle shuts off. Me and my children’s safety is at risk due to it stopping multiple times on freeway also stopping at a stoplight that was around a curve with blind spot . Has stopped on a two lane road with no emergency lane. Stopped at another light and had to wave traffic around.

I would like to submit a formal complaint regarding the inadequate rental reimbursement being provided while a 2022 Chrysler Pacifica is undergoing recall repairs (Recall No. 226 – Ejection Mitigation). I took my vehicle to the dealership for recall repairs on March 12, 2026. The dealership does not provide loaner or rental vehicles. Chrysler Customer Care representatives advised that rental reimbursement would be limited to 60 per day. However, the actual cost of obtaining transportation exceeds this amount. Even a significantly smaller compact vehicle currently costs approximately 69 per day to rent. A comparable vehicle to the Chrysler Pacifica, such as a minivan or similar family-sized vehicle, is substantially more expensive than the reimbursement amount being offered. As a result, a compact vehicle that is significantly smaller and not comparable to the Chrysler Pacifica had to be rented. This vehicle does not meet the same functional needs, comfort level, or standard of transportation normally relied upon. Additionally, the difference between the rental cost and the reimbursement limit must be paid out-of-pocket. This situation creates an unfair financial burden resulting solely from a manufacturer recall that is outside of the consumer’s control. Customer Care has been contacted multiple times regarding this matter; however, no resolution or adjustment to the reimbursement limit has been offered. The current policy places additional financial strain on the consumer during the recall repair process. Assistance in reviewing this matter and encouraging a fair resolution would be greatly appreciated.

The headrest failed. The headrest pivots on the steel supports, and there's a ratchet inside, that is actuated by a plastic piece, which is then connected to cabling. The plastic piece internally broke it's actually unbelievably small for the application, the engineers should be ashamed of themselves. I'm going to guess 1/8" diameter, maximum, snapped right at the ratchet mechanism. If the weak plastic does happen to let loose in an accident, that headrest is going to flop forward and will not provide the head support that it's supposedly designed to provide. The way the headrest angles forward I worry about decapitation or severe neck injury causing quadriplegia in the event of an accident. This piece should work and be dependable not a tiny piece of plastic that often breaks with minimal use. We should know we have dependable headrest function in the event of an automobile crash.

The car randomly gives a warning on the dashboard in red, with red turtle symbol, to pull over and stop safely. A lot of the time we will be on the highway and can't pull over safely. It happens at any speed. 25, 80, 65, etc. It's been happening since we got the vehicle in 2023 and no dealership understands what the issue is even though there are clearly recall repairs for other same make and model of other years surrounding this 2022 model. They keep saying its Keysense but its not locked for speed and that doesnt make sense because it doesnt happen at a high speed only, it happens randomly even in town going slow. They've done the "software update" a fee times, but it's clearly the same problem as the others with the wiring harness. That part needs to be replaced because the patch software fix isn't a solution and its definitely not a safe solution at that.
POWER TRAINFebruary 22, 2026
I was driving on the highway when an alarm sounded and warning message displayed: "Stop Safely, Vehicle Will Shut Off Soon" At the same time, a turtle icon came on, along with the check engine light. I lost all power in the gas pedal. I pulled over, and after shutting my van down for a few minutes, I was able to turn it back on and get to my destination. However, the same exact issue happened on my way home. That time, I had to pull over -- still on the highway -- during rush hour and I did not feel safe as other cars zoomed by me. I often travel with my show dogs, which was my main reason for purchasing a new van. Naturally, I am concerned about their safety and well-being too. After this happened, I took my van -- which has less than 36,000 miles on it -- to my local dealership. They performed a software update and sent me home. I took my van on a "test ride" and the same alarm sounded and warning message displayed. I brought it back to the dealership and they investigated the problem further. I now have to replace the transmission -- a $7,000 repair -- on a van that is less than five years old with very low milage. I had NO prior issues, odd noises, or warnings indicated by my van.

Driving on the freeway vehicle suddenly enters limp mode with stop safely vehicle will shut off soon displayed. Transmission was replaced once already for this issue, a year later and I am experiencing the same thing. Very dangerous to lose all power while going 80mph in the third lane of a three row freeway and have to attempt to get all the way across lanes of traffic to safely stop while losing all power. Family vehicle loaded with kids. Has entered turtle mode 3x on the freeway after transmission was already replaced once. If I drive under 65mph it goes fine, but who wants to drive behind semis all the time on the freeway? Untrustworthy vehicle.

The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving into California through a storm with the temperature at 39 degrees, in heavy rain and wind, the defrost button was pressed, and the windows immediately fogged up. The contact cleared the windshield with a cloth while attempting to navigate through traffic. The contact became aware that the defogger had caused the failure. The local dealer was contacted, who was unable to determine the cause of the failure. The vehicle was not diagnosed or repaired. The manufacturer was contacted, but the contact was unable to speak with a representative. The failure mileage was 30,000.
SERVICE BRAKESFebruary 10, 2026
Brakes have a skip feeling horribly. We took to garage after changing brakes and rotors 2x to be sure it wasnt the issue. But doesnt even feel safe to drive the garage took the whole front end apart down to the calipers and didnt find any issues so they questioned if maybe it wasnt a similar recall to the hybrid braking issue to do with the caliper? The van only has 45,000 miles and feels so unsafe to drive anywhere with 5 kids and a newer vehicle you shouldn't not feel safe driving. Ive seen more than my complaint on brake issues just by looking around on internet and the brake light will come on I have to turn off and crank vehicle again for the red brake dot to go away and engine light comes on.
SERVICE BRAKES,FORWARD COLLISION AVOIDANCEFebruary 4, 2026
In May 2025 shortly after purchasing this vehicle I had complete Brake failure while driving down the road. I was driving approximately 65 mph when the forward collision warning, ABS warning, Brake Warning, Auto Emergency Brake warning lights came on and when I pushed on the brake pedal it was hard as a rock and wouldn't stop the vehicle. It was returned to the purchasing dealership, and they replaced the Brake Booster. On November 27th of 2025 I was driving down the road and the exact same problem occurred with a complete loss of brake function. It went to the local Chrysler Dealership; they replaced the Master Cylinder. I picked it up 12/10 drove it 5 miles home and parked it. I got in the vehicle the next morning to go to work and on my way to work all the same dash lights came on and I had a complete loss of Brake function. It was towed back to the dealership again; this time they replaced the ABS module. I picked it up on 12/30 drove 5 miles home, parked it, tried to leave 3 hours later and couldn't push the brake pedal far enough to get the van started. When I did finally get it started all the same dash lights were on and the brakes were not functional. It was towed back to the dealership again and this time they replaced the wiring Harness. I picked it up 1/16. Drove it minimally over the weekend, on 1/19 the front-end collision warning lights came on and the check engine light came on while driving 70 mph. The brakes didn't lose function this time, however. The following day I was sitting in line at an elementary school waiting to pick up my daughter when all the same dash lights came on again and I had a complete loss of brakes, it was towed back to the dealership, where it still is. There was contact made with Chrysler engineering during the 2nd repair. The dealership has been able to recreate the problem more than once since they have had it. This put myself and my children a significant risk of injury, and other cars if there was an accident.
VEHICLE SPEED CONTROL,ENGINE,FORWARD COLLISION AVOIDANCEFebruary 2, 2026
A message appeared on the dashboard stating the engine would shut off soon. I could not accelerate. This has happened multiple times (at least a dozen since I bought the vehicle in April 2025) and I cannot drive above 70 mph or it will cause the issue to occur. Several times I have been in multi-lane traffic and because you cannot accelerate, I have to try to park on the should across multiple lanes of traffic at high rates of speed while slowing down.
ELECTRICAL SYSTEMJanuary 27, 2026
The vehicle experienced a failure of the hybrid battery thermal management system. A diagnostic trouble code (P0E15 – Hybrid/EV Battery Pack Coolant Heater “A” Control Performance) was set and the check engine light illuminated. This component is responsible for regulating high-voltage battery temperature and is part of the hybrid cooling/heating system required for safe operation and emissions readiness. The failure occurred at approximately 100,700 miles on a 2022 Chrysler Pacifica Hybrid. The battery coolant heater is a sealed, high-voltage electronic component with no maintenance or wear interval and is not a consumable part. Loss of proper battery temperature control can affect hybrid system operation, charging, regenerative braking, and power availability, and can potentially result in reduced power or limp-mode operation as the vehicle attempts to protect the high-voltage battery and inverter. The problem was confirmed by a Chrysler dealership and an Area Service Manager using factory diagnostic equipment. The fault code is stored and permanent. The vehicle was inspected by the dealer, and the component was diagnosed as failed and requiring replacement. No accident, misuse, or maintenance neglect was involved. The manufacturer declined warranty or goodwill coverage based on internal policy, despite the failure occurring immediately beyond the hybrid warranty mileage threshold and involving a non-wear, safety-critical hybrid thermal management component. The repair estimate is approximately $1,900 out of pocket. The warning indicator (check engine light) first appeared at the time of failure. The component remains available for inspection and replacement by the dealer. Also the dealership did not provide is with a written justification for the denial and corporate (Stellantis) stated that they were not able to provide justification either other than what the automated prompt was giving them (that the part was not covered under goodwill)

While driving 75MPH, the vehicle shut down with a message to pull over. All acceleration was lost. We had to navigate, without warning, across 3 lanes of traffic to pull over on the right side of the highway. This was EXTREMELY unsafe as we were slowing down and essentially cutting off other vehicles who were honking at us as this was a traffic hazard. All 6 members of my family were in the vehicle. We had this happen another 3 times before arriving home. I took it to my local dealer, who could not replicate it. They performed a software update, which I have since learned would NEVER fix this problem. We had it happen to us 5 more times, so I took it to another dealership, who initially could not replicate it. In all cases, we had the following check engine light codes be triggered. I have screen shots of them from my personal OBDII reader: P0A43 "Drive Motor 'A' Position Sensor Circuit Intermittent", C2225 "Status: MIL - PCM Disabled EDU: No Sub Type Information". After doing my own reading, it seemed many others have experienced the exact same symptoms and it has required a transmission repair/replace. I also learned most people had a battery charge on the high voltage battery (as this is a plug-in hybrid) when the vehicle shuts down. I charged my van, took it for a ride at 75MPH and it occurred within the first mile. I took it back to the dealership, and at this point, I refuse to drive it until the hardware fix happens. It seems like this should be a recall for all vehicles that can have this problem. It is EXTREMELY unsafe as it occurs at 70MPH+ speeds.

The vehicle was running fine when it suddenly shut down while driving. It was started back up 3 times before it would not start again.it was towed. We were told the two batteries needed to be replaced so we had it towed to midas because they had recently changed the auxiliary battery so it was under warranty. They replaced they replaced thr batteries but when they went to started up, thr engine started knocking and wouldn't start up again. Tr engine needs to be replaced.

The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving approximately 35 MPH, the vehicle independently accelerated. The contact stated that several unknown warning lights illuminated. The contact was able to stop the vehicle by depressing the brake pedal; however, when the brake pedal was released, the vehicle independently accelerated without the accelerator pedal being depressed. The contact stated that the accelerator pedal failed to respond while being depressed. The contact stated that the failure had occurred three times and occurred while driving at various speeds. The contact stated the failure also occurred while driving at 70 MPH. The vehicle was taken to an independent mechanic who was unable to retrieve any fault codes but informed the contact that the 12-Volt battery was not at full charge. The vehicle was taken to a local dealer, where it was diagnosed, and determined that the accelerator pedal assembly and the main computer had produced fault codes, but the dealer later stated that there were no codes. The vehicle was not repaired. The manufacturer was informed of the failure and a case was opened but was later closed. The failure mileage was 41,607.
FUEL/PROPULSION SYSTEMNovember 26, 2025
Fuel line leak near engine. It sounds very similiar to the 2023 Chrysler Pacifica Recall. The vehicle reaked of gasoline fumes coming in through the vents. Our mechanic replaced the fuel line, said it had a hole. We felt very unsafe even starting the car when it reaked of gas due to risk of fire. No warnings lights or anything, just the smell to tip us off. No one else has inspected the car since we didn't have anything catastrophic happen.

ELECTRICAL SYSTEM,ENGINEOctober 14, 2025
The vehicle’s electrical or engine system appears to have failed, causing a sudden loss of power while driving. The exact component has not been identified by the dealership or Chrysler, but it may be related to the recall affecting the vehicle’s powertrain control or electrical system. The vehicle is currently at the dealership and available for inspection upon request. The issue has created a serious safety risk. The vehicle has repeatedly lost power and shut off while driving on the highway, including one incident where I was unable to fully pull off the road before it stalled in active traffic. My children were in the vehicle at the time, placing us in immediate danger of being struck by oncoming vehicles. Because the vehicle can stop without warning, it is unsafe to drive and puts both my family and others on the road at risk. The dealership has confirmed the issue and advised that parts are on backorder for at least 10 weeks. They also stated that even once the parts arrive, they are uncertain how to complete the repair, indicating that Chrysler has not yet provided an effective fix. The vehicle has been inspected by the authorized Chrysler dealership multiple times, and the issue has been reported to Chrysler. It has not been inspected by police or insurance representatives. A warning message reading “Pull over as the vehicle will stop” appears on the dashboard prior to each failure event. The first occurrence was on June 10, 2025, while driving on the highway to my daughter’s graduation. Since then, the same warning has appeared multiple times before the vehicle shuts off completely. It has been 6 weeks at this point and I have no vehicle. Chrysler has offered rental reimbursement of $60 a day but there is no vehicle that is a 7 passenger like my van that is of this cost. At this time they do not have a timeframe of when the vehicle will be fixed and denied a buyback or replacement.

Dash message "Stop safely vehicle will shut off soon" included a red exclamation mark in the middle with a red triangle and a turtle in the bottom right. A yellow reduced speed type indicator in the top right and a check engine light all at the same time. Also a chime sounded. This occured in town moving around 35 mph. Dealer stated this exact VIN had already received a rebuilt trans around 50k miles from another dealer. Took dealer 2+ months to repair, and had to call many many times to encourage them to push to get it completed. Manufacturer seemed to delay sending parts because they can - no good reason provided. First Power Inverter Module (PIM) was replaced, but that did not resolve the issue. Then dealer rebuilt entire transmission. Dealer lead tech stated that a roller bearing near the bottom of the trans appeared to have start failing, which then causes a short and or some kind of damage with the electrical "phasers". Multiple conversations with lead technician and Chrysler cares indicate clearly that Chrysler is NOT investigating the issue deeply (they did not want any of the failed components, nor did they ask for details from dealer - are they even trying to resolve root cause.... this has been occurring since the earliest models). Chrysler should have a financial incentive to fix to remediate warranty repairs. Why can't chat GPT find literally anything on the web about the root cause, why is it nothing other than reports of the issue can be found??? Nobody has done a root cause analysis. Is Chrysler paying to minimize bad press??? If Chrysler replaced entire trans with rebuilt one at 50k and now it failed again at 80k - when can I expect it to fail a third time?

While driving several warning lights came on and the transmission shutdown. I coasted to the side of the road. The engine kept running but the van would not move. I shut the engine off and after a few minutes (5 to 10) I restarted the van and the warning lights were on on. I drove the van 2 or 3 miles and it happened again. Once again after a few minutes all the warning lights were off except the check engine light. I returned to my motel and called a local Chrysler Dealer. I then was able to drive to the dealer with no problem. When they diagnosed the problem they said it had several codes. The following is what the service department did. “Digit-verified cel stored codes p0973 in Tim and in pcm, abs bcm u0418 u0402, u422, alll are stored erased codes, no updates, removed air box to access ground and Tim connector, inspected harness from Tcm to transmission, removed and cleaned ground on trans, reassembled and test drove could not duplicate concern. After more test drives still unable to duplicate concern.” After this I was able to drive home 1500 miles without any problems. Not sure if the problem is fixed or not. I just wanted to report this just in case others are having the same problem and to build a case for a recall if needed.

Multiple vehicle shut downs while driving on the highway. A red turtle icon appears on the instrument panel letting me know vehicle will be shutting down, and I will have to pull over. This has happened to me over a dozen times, and I have had the transmission replaced twice now by the dealershp. This is a very unsafe situation because if you are on a road with no shoulder you will stop in the middle of the road and be at risk for a collision. This can happen anywhere, and happened multiple times when I was driving my family for a vacation hours from home. One time it happened I was with my disabled daughter and we were stuck by the side of the road miles from home. When that happened, I could not get the vehicle to restart. If it had not been for AAA I don't know what I would have done to get home. This has happened to me a dozen times since 2023. Chrysler has known about this problem for years! My wife was driving our 2022 Chrylser Pacifica when it died in the middle of an intersection in downtown Austin. She was in the turning lane waiting for the light to change when the auto stop/start feature activated. As the light turned green and she went to proceed, the car started up and then cut back off while simultaneously placing itself in park and all of the lights on the dash began blinking and messages stating "service shifter", "service parking brake", "service brakes" etc started popping up on the dash. She tried to restart the car and nothing happened and the emergency flashers would only work intermittently before shutting off and randomly coming back on for a few seconds minutes later. Eventually a police officer pulled up behind her to block traffic while she and they tried to get the car started. When I arrived, I was able to jump the car and get it out of the road and to a nearby parts store to check the batteries. Oreillys and Advanced Auto confirmed that the alternator, batteries and starter were good based on their diagnostic scanners. She was able to drive it to San Antonio and back the next day with no issues, but that night, when we went to dinner and stopped for gas, it did it again while in the fuel line although I was able to restart the car this time but now displays "Stop/Start Unavailable, Service Stop/Start". After the second event, went back to Oreillys and had it scanned, there were no primary diagnostic trouble codes, but 8 stored additional ABS trouble codes relating to low voltage, and electronic failures across multiple systems. This all came after having the oil changed and a safety inspection performed on 23 July in which everything passed, but they recommended a coolant and brake fluid flush. Concerned about this happening in congestion on a highway or somewhere with limited options for recovery.

6/21/25 , a Saturday, I stopped at a traffic light when gas saver function kicked in and shut car off. The car went into park mode by itself and parking brake engaged. The car would not start and had to be towed. It sat until Monday when I was able to start it and drive it to my mechanic's business a mile from my location and shut it off. Immediately after shutting it off it would not start again. My mechanic was able to start it a few days later and when he drove it about 5 miles it just shut off and wouldn't start again. Today he is replacing the starter and believes the gas saver function may have contributed to the increased wear on the starter leading to failure, as well as the heat from the catalytic converter located near the starter. I am very nervous about driving this car again and worry it may happen again when I am in traffic. This is a major safety problem if not resolved.
SEATSJune 27, 2025
Part 68324912AC Latch Release Cable failed on both middle seats within one week of one another and approximately three weeks after the manufacturers warranty expired on the vehicle. The result is that both seat backs do not latch into place vertically and the entire seat freely slides forward and backward, slamming backward on acceleration and forward on braking. We have read on Internet forums that this is a very common problem. The left side seat failed while my daughter was riding in it and the seat back released. If this had happened during a collision, it could’ve been catastrophic. Upon stopping the vehicle and exiting, we found that it slid freely forward and back as well. The other side failed while the vehicle is parked, and we released the seat back lever. The seats are unusable as a result. If the failure had happened while driving or impact, it could have been devastating as the seat would have slid independent of the seatbelt and the back slammed forward driving the passenger’s head into the back of the front seat. A Chrysler dealer diagnosed the problem charging us $250 for the diagnostic fee and quoted another $750 for the repair. Parts cost about $30 each. There were no warning indicators prior to the failure.

The concern is that in a crash, the passenger seated in the driver's-side, 2nd row seat will not have adequate protection from a neck injury due to the protruding headrest.

First of all, I would like to say that I received an "IMPORTANT SAFETY RECALL" notice citing that "certain 2022 Chrysler Pacifica vehicles failed to perform" However, my car's problem, though similar, is not quite the same as the problem mentioned by the Federal Motor Vehicle Safety Standard" - rear visibility. My car's DRIVERS' SIDE VIEW mirror is the one that poses the problem. When I put the vehicle in reverse, the right-side view mirror adjusts so that I can see the right backside of the vehicle. This, unfortunately, does not occur with the left sideview mirror. I can only see the ground. This an obvious determent to the safety of not only myself, but others as well. When I reported this irregularity to Hunter Dodge, I was told that was just the way the mechanism worked. I was told that there was nothing they could do. No one bothered to check it out. No one seemed concerned. This has been an issue since I purchased the vehicle. I don't know if anyone else has this same issue; but I feel that I am not the only one. In 2024, our vehicle displayed a critical warning stating: “Stop safely. Vehicle will shut off soon.” While driving on the highway, the vehicle began losing engine power—a terrifying and dangerous experience. We immediately took it to the dealership, where they diagnosed a transmission failure and proceeded to replace the transmission. Unfortunately, we are now experiencing the same issue again this year. The vehicle continues to lose power and shut off while in motion on the highway, creating an extremely hazardous situation that nearly resulted in a serious accident. This ongoing malfunction poses a significant risk to the safety of drivers and passengers. Chrysler must take immediate responsibility by recalling these vehicles and replacing them with safer alternatives to prevent further danger to human life.

Subject: Urgent Safety Concern – Chrysler Pacifica Hybrid (VIN: [XXX] ) Dear NHTSA, I am writing to report a serious safety issue with my 2022 Chrysler Pacifica Hybrid (VIN: [XXX] ). The vehicle experiences sudden power loss due to the following error codes: P079A-00 (transmission issue), P0A43-00 (hybrid battery issue), and P0A7D-00 (battery voltage issue). These problems have nearly caused accidents, including: On the highway: Power loss while overtaking, leaving me in a dangerous situation. In city traffic: Repeated power loss, almost causing collisions. The issues are well-documented among Pacifica Hybrid owners in the USA. NHTSA has received numerous complaints, and there are active class-action lawsuits due to unresolved problems. My vehicle was involved in a minor collision in the USA, classified as "clear title" (cosmetic damage only, no impact on the drivetrain or battery). Repairs were done professionally, and there is no connection to the current issues. I have contacted Chrysler and the authorized service center in Berlin, but received no response. This issue poses a direct safety risk, and I urge NHTSA to investigate and ensure a permanent solution. Attached: Error codes and incident descriptions. Correspondence with Chrysler and the service center. Documentation of the collision and repairs. Thank you for your attention. I look forward to your prompt response. When driving on the interstate highway with about 70 mph a warning message “Service hybrid electric vehicle system” appeared. The high voltage battery charging level was less than 1%. About 30 minutes later, after exiting the highway, another warning message was displayed: “Stop Safely Vehicle will shut off soon”. After about 30s later the motive power was stopped and the car continued rolling. There was no propulsion power anymore, and I was only able to roll into the entrance of a parking lot. Luckily it was a relatively safe location where the vehicle came to a stop. I was alone in the car and was not in direct danger. The vehicle could not be restarted. I called a towing service, and the car was transported to a Chrysler dealership. The fault codes were read out. Chrysler Star recommended to replace a 350 A fuse. The dealership service team drove the car for more than 70 miles without being able to reproduce the fault after this repair. One day after collecting the car from the dealership I drove about 300 miles on the highway. After about 250 miles the warning message “Service hybrid electric vehicle system“ appeared. The high voltage battery charging level was less than 1%. When driving on the highway with about 55 mph, about 150 miles after the warning message appeared, the motive power stopped inadvertently without any warning. I felt a short impulse on the gas pedal, then while rolling at below 50 mph the message “Switch P to start engine“ was displayed. I managed to roll out from the 3rd lane from the right to the emergency stopping lane. It was a dangerous maneuver and I feared for the safety of my five family members traveling with me in the car as the oncoming traffic behind my car didn’t slow down even after switching on the hazard lights. The vehicle could not be restarted and I had to call a towing service to transport the vehicle to the nearest Chrysler dealership. Now I am waiting for the service team to start reading out the failure codes.

It was a rental car, driving on freeway , warning light will come up , saying engine will shut off. The Van had no power, and I had to pull over. I turned off the car for 2-5 minutes and restart. Then the car will accelerate again. I had to drive from LA to Las Vegas. It started doing that after Loma Linda , CA [XXX] , on [XXX]). It took me 7 hours to get to Las Vegas. Because of the constant shut off warning. I had no choice but to do this because SixT rental car couldn't get another car to me for 5-6 hours wait. Shortly into my trip home from work, I entered onto the interstate and accelerated up to speed. Within a minute the vehicle seemed to slow down and I had no power using the accelerator Pedal. The vehicle continued to slow and the gas engine failed to turn on.(this is a PHEV hybrid model). Eventually the vehicle lost all power and I was able to park it along the side of the interstate. The vehicle was still on, AC and electoral systems worked. After about 5 minutes, gas engine spontaneously started back up. I was able to gain speed start bad down the road. Approximately 3 min later, gas engine shuts down and same process occurred. This cycle took place about 6 times before vehicle would not start up anymore and ultimately had to be towed. After a week, dealership is not sure what’s wrong as the transmission has been replaced which did not fix the problem. Dealership is now planning to replace the PIM module although there is an over 45day wait and they are unsure if that will fix the problem.

I am lucky that I was not on a major highway at the time as I had three young children in the vehicle. Once stopped I was unable to turn off the van. The vehicle start/stop button was not responding and it took a long time before I was able to turn off the vehicle to try to restart it. The message remained and the car had to be towed to a dealership. It is now with the dealership while they determine the cause of the shut down. This is a serious issue to occur on a relatively new vehicle.

While driving on the highway in rush hour traffic I got a message to pull over when it was safe to do so, and the car started losing power. The breakdown lane was in use for traffic, so I had to drive 1/2 mile to find a safe spot with the car going much more slowly than the flow of traffic. I turned the car off, and opened the door to 'reset' it, then nervously pulled back out into traffic. I was able to drive to about a mile from my house, and I got the message and lost power again. I go the car to my mechanic, who said that the car had transmission failure. I had the car towed to the dealer, who confirmed as much.

On three separate occasions (December 2023-February 2024), while driving in temperatures below freezing, our 2022 Chrysler Pacifica Hybrid dashboard gave the following warning, along side the illumination of a red turtle warning light: "Stop Safely. Vehicle will shut off soon." The vehicle then lost the ability to accelerate, and the speed dropped significantly and immediately. Two of the three times, my one and three year old were in the vehicle and there was no shoulder on the 50mph road we were traveling on after sunset. Fortunately, the three times this happened, we were less than a half mile from our home and we coasted downhill to the bottom of our driveway. Our vehicle has now been at a certified Chrysler dealership for three full business days, and the technician "has no idea what is wrong," "is still working on it," "has no idea how long it will take to fix," and "has no rental or loaner cars available" for our family while we wait for our family's mode of Transportation to be returned. They claim they are unable to recreate the issue, so they cannot fix it. We showed them a picture of the dashboard, but that is not enough for them. It is currently 72 degrees in Dayton, OH, and the issue has never occurred in temperatures above 30 degrees. Thus, I doubt they will be able to "recreate" the issue until next winter. They've offered no other options or resolution until they can "recreate the issue."

Ever since delivery of our vehicle it would seem to have a hard shift between the electric and gas motor. Recently, we were driving one morning and noticed that when the gas engine kicked in, the RPM’s were much higher than usual. Later that day we received the “Stop Safely the Vehicle Will Shut Off Soon” message along with a check engine light. Shortly thereafter the vehicle went into limp mode. We are very lucky this happened in town as opposed to on the highway or in a remote area where this would pose an even more serious safety risk. Especially with young children on board. We then brought the vehicle to the nearby dealer where they found P0A43-00 “Drive Motor "A" Position Sensor” and C0063-9A “ Yaw Rate Sensor-Component or System Operating Conditions” codes present. After nearly a week of troubleshooting and opening up a “STAR” case, the dealership is now in the process of acquiring and replacing the transmission on our vehicle. This seems to be related to the recent recall 03A “Loss of Motive Power” and Chrysler TSB 21-008-23 affecting the internal transmission wiring connector. Our recall was completed promptly after issuance but this did not address the underlying issue of the internal transmission wiring connector. Our van is just over one year old and has under 21,000 miles on it. Needless to say this is unacceptable for a vehicle of this age, use, and especially MSRP! We purchased this vehicle as a safe option for our family and now we have lost confidence.

FORWARD COLLISION AVOIDANCEFebruary 15, 2024
While travelling on a controlled-access highway at 70 mph with the Adaptive Cruise Control active, and no other vehicles or objects nearby, the braking system engaged spontaneously (without driver input) and forcefully, for approximately .5 seconds, also engaging the anti-lock brakes. The rapid deceleration was forceful enough to snap my family of 5 all into their seatbelts. The speed of the vehicle slowed approximately 10 mph or more during this incident. My family and I were fortunate enough that no vehicles were behind us, no collision occurred, and I was able to maintain control of the vehicle though I was very much surprised. No warning indicators nor messages appeared. I took the vehicle to a servicing dealer three days after the incident for inspection/diagnosis. I was told be the service agent at the desk that, in order for this to not happen again, I should not use the Adaptive Cruise Control. The technician, who was at the service desk, stated he was unable to reproduce the incident on test drive, and that he recalibrated the Adaptive Cruise Control system and sensor, without further explanation as to what that meant.

While driving on the expressway at expressway speeds we got a message on the information system stating the engine will shutdown soon, pullover safely as soon as possible (paraphrased). The van started to lose power almost immediately upon receiving the message. We were lucky to pull over on a multi lane expressway since we were already in the right lane. It appears the gas engine did not engage when the battery was almost out of power (1%). On the side of the road, it took two times to turn the van on and off before the gas engine started. We were able to make it home. The next business day we contacted the dealership and had it towed there for service. After evaluation I was told the entire transmission needed to be replaced since a generator for the electric motor is housed there and is apparently the cause of the problem. At this time my vehicle is still with dealership awaiting parts. I did research online and see the message we got was the focus of a recall (to display the message) but the cause of the mechanical\electrical failure was still unknown from Chrysler/Stellantis. This is unacceptable. We could have easily been in a crash due to the issue. If they can't fix the actual problem, the message needs to display much sooner while the battery still has sufficient power such as 10%. I believe we had 1% indicated on the driver display at the time of the message.

Prior warning: On 7/11, dash showed "put in P to park" alternating with "press brake and push button to start" while driving down the road (note D for drive in lower right on attached images, also photo of car still on/in run). This started at the same time the car bucked and the check engine light came on solid. While waiting and thinking about what to do next, the car restarted on its own. (It is possible I pressed button 2x to turn off as I recall it didn't react. But restart was a full minute later!) Restarted. Dealership seemed unconcerned, despite the known issue with Pacifica hybrids. Same issue occurred 7/12. On 7/13 the radio rebooted while driving down the road with no warning. The dealer originally took a look at it on 3/13/23 and determined that the fill line was just loose into the tank. However when we filled the tank with gas again on 3/20/23 the problem began again. It went back into the dealer on 3/21/23 and they are replacing the fuel tank and the fuel fill line. No warning lights or messages ever came on.

Our Chrysler Pacifica hybrid had the software update related to the wiring short that has caused multiple Pacifica Hybrids to shut down. We had the recall "repair" on 3/9/2023. On 3/12, while driving on the interstate traveling 70mph, the warning popped up, indicating that a power shutdown was imminent. I had my entire family, kids, and elderly parents in the car with me. Fortunately, we were able to pull over safely, but had we been on the road during a busier time, we would have been in serious danger! Our Pacifica has less than 10k miles and has had no problems previously. The software "fix" was not a fix at all, and this is a huge concern since the entire reason for purchasing a van was to travel safely and conveniently with our children.

On 3/6/2023 while driving on a freeway, my vehicle experienced a complete loss of motive power and the accelerator was unresponsive. The dash gave me a warning that the vehicle was off. On 3/7/2023, the same incident occurred twice while on the same freeway. on 3/7/23, I dropped it off at Moss Bros Chrysler Dealer in Riverside, CA for the loss of motive power recall I was just notified of. The software update from the recall was performed, and the service adviser informed me that it was successful and the issue had been resolved. On 3/9/2023, I drove it on the freeway, and again, the same loss of motive power occurred. This time, the dash warned me that vehicle shutdown was imminent, and a turtle icon displayed on the dash. However, the moment the dash displayed this information, all motive power was lost and the experience was identical to before the software update. This is a significant safety issue as the freeways in my area usually do not have any shoulders or areas that are safe to pull over on. This is a serious defect that poses a safety risk to anyone in the vehicle. It has not been fixed by the recall.

While traveling at full highway speed, the Pacifica turned off entirely without warning. We had to navigate in traffic through 4 lanes to pull over with children in the vehicle. The vehicle would not turn back on after pulled over until hours afterwards. However we towed the vehicle to a dealership due to the safety concern

I purchased this van brand new. On the day I picked up on a Saturday and very excited! Sunday I went to go out and it didn’t start! Called the dealer on Monday and they sent out someone to jumpstart it and it wouldn’t start. It was towed to dealer and it was told it was the pcm that needed to replace. Work was done and it seemed fine. As I was driving on a trip from Chicago to Vegas with my grandchildren on two and a half day trip! The navigation system would shut down and reboot this has happen 5 times while driving. Making me lose my route. Then one day about two weeks ago it wouldn’t start or lock. After numerous times finally got it started but it wouldn’t lock so I and drove it to dealer. They hooked it up and could not find anything on the monitor. Then drove it back home and the same thing happened! So again I drove it to the dealer and again nothing showed up! This is very scary that it is happening and no explanation. The dealer says if we can’t recreate it then we don’t know what is going on! I need help on finding out why this is happening.

The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at 80 MPH with the cruise control activated, the vehicle experienced phantom braking. The following day, while driving at an undisclosed speed, the vehicle was vibrating and the brakes failed to immediately stop the vehicle. The contact stated that the brakes caught fire, and there were flames coming from the brake calipers. The contact was able to put out the fire with a hose. The vehicle was towed to the dealer, where it remained for twenty-one days; however, no cause of failure was found. The failure recurred, and the vehicle was taken to an independent mechanic, who recommended the contact stop driving the vehicle as it was unsafe on the roadway. The vehicle was not repaired. The manufacturer was made aware of the failure and dispatched an inspector to the dealer to inspect the vehicle, but the report was not yet available. The failure mileage was approximately 13,000.

Please investigate the matter as these are major safety issues.

I was driving on freeway today January 13, 2023 and vehicle lost all propulsion (did no respond to accelerator being depressed) with warning message on screen "Press Brake and Push Button to Start" and "Shift to P to Start" while at 64mph. Vehicle was slowing down and would not accelerate when pushing on the accelerator pedal. Happened twice during 20 min drive into work. I was able to have my phone ready and took pictures of the warning screen the second time. Pulled off to the side of the road to be able to switch the car to "P", turned off the the car with the push button and then restart. Traveling down a country road on a clear afternoon, the vehicle traveled approximately 35 mph while cresting a small hill. On the other side of the hill was a "T" shaped intersection where another vehicle stopped in the intersection from a stop sign several feet behind. The other driver failed to complete the turn when they saw oncoming traffic. The driver noticed the intersection had an obstructed path and attempted to move into the oncoming traffic lane since no oncoming traffic was visible. Just then, the onboard computer installed in the control panel of the drivers seat had acted upon either the obstruction or the switch in lanes and assumed complete control of the braking and steering systems. The driver was unable to retrieve control from the computer systems artificial intelligence before a direct crash occured with a telephone pole. Rendering the vehicle dangerous as live wires draped the saturated earth below the tires and feet of passengers exiting the vehicle. The manufacturer was contacted, but no follow up has been made. Neither the police or insurance representatives have been helpful or any investigation made into this account. There were several lights, sensors, noises, and messages that were flashing and beeping but none were able to be understood before the sudden impact.

When it is unusually cold, the heater fails to heat the hybrid battery sufficiently to register the correct voltage. This causes the car to enter “turtle” mode- a special mode that limits the car to 30 mph. It’s designed to allow the driver to get to a dealership. When we were driving at highway speeds about 70mph, during a particularly cold day, the turtle mode dropped us suddenly to 30 mph. This nearly caused an accident as we almost lost control of the vehicle on ice and snow. It was so sudden we had to swerve into the side of shoulder.

22' Pacifica has advanced front brake assist . It does not work. I have tried and it only sets off a audible warning noise and dash says BRAKE. NO braking assist like it was stated it would have. ENGINEOctober 28, 2022
Driving down the highway at 80 mph, the engine suddenly quit. The check engine light and battery light illuminated after the engine had quit. I was able to coast to the shoulder. Once stopped the ignition could not be turned off to try to restart the van. It would only move from Run to ACC. After sitting without touching anything for 4-5 minutes, the Ignition indicator moved to Off and then was able to be restarted. This is the third occurrence, the first happening while stopped at a red light and the second in a parking lot when shifting from reverse to drive. The dealer agreed to pick up the van from my work and drive it to their service center. The van quit on that driver once on the return trip. The van had just been in service for 36 days and was returned as operational with no defects on October 19, 2022. The parking lot event occurred 5 days later on October 24 and the highway shutdown on October 26. This is obviously a major safety risk. If I had not been driving at 0500 with minimal traffic the outcome could have been worse.
VEHICLE SPEED CONTROLOctober 10, 2022
On Oct. 9, 2022, at approximately 6:30 p.m. EDT, I was braking as I approached a stop sign. For the first second or two of braking, the van slowed normally. Then, as the van was nearly stopped at the stop sign (probably 10 feet ahead of it, and approx. 5 mph or less), while my foot was still gently depressing the brake pedal, the van began to accelerate on its own. In response, I stepped down hard on the brake, lurching the van to a stop. After the hard stop (and explaining it to my family), I gently accelerated through the intersection and parked the van in my driveway. (The stop sign was 2 houses from home.) This was on a residential street, 25 mph speed limit, clear day, dry pavement. Before applying the brakes I had been coasting down the street between 20 and 25 mph. There was no other traffic and no pedestrians. When the van accelerated on its own, we had nearly but not completely come to a stop yet. My foot was still gently depressing the brake to come to a smooth stop. Neither the accelerator nor the cruise control was engaged. With unwanted acceleration while the brake is depressed, I do not think the vehicle is safe to drive. Today (the next morning) I am in the process of calling to have it flatbed-transported to the dealer for inspection. The van is brand new; we've had it 5 weeks, it has 698 miles. No problem or warning prior to this.

On more than one occasion when I was on a four-lane highway with no vehicle in front of me in my lane, while I was passing a slower moving vehicle in the outer lane, my vehicle slammed on the aes brakes and would have absolutely caused a crash if there would have been a vehicle behind me. That person could have no possibly reacted fast enough to prevent his vehicle from rear-ending my vehicle. Again, there was no one in front of me, only the vehicle in the outer-lane. As far as the rear-cross-traffic problem, my vehicle is one that is missing Chrysler's Sphere and rear-parking-assist because of the chip shortage. There aren't any sensors in the back bumper at all and I'm not sure what triggers the rear-cross-traffic alert, but it does not alert until I've backed up so far from my parking space that if there was a car coming quickly it would surely hit me. This system does not work any where as quickly as any other car I've had it on in the past.

Was driving home and accelerating when there was a mechanical "thump" noise and the vehicle MIL indicator came on with a "red turtle" indicator on. Vehicle speed/performance was immediately reduced and I was not able to maintain a safe speed or have enough power to travel at appropriate speeds when hills were involved. This is the second time this has happened in this vehicle. The dealer has already looked at the vehicle prior and claimed they found the issue and resolved it. Since it occurred again the issue has clearly not been resolved or properly identified.

All engine power and speed control was lost at highway speeds and was barely able to get out of the way of traffic. I was able to coast to the shoulder without injury or accident with a few near misses. Opened / closed the hood at the side of the road and the fault didn't clear. 5-10 minutes later vehicle cleared the error and I was able to drive to a local dealership who told me "there is nothing they can do for me". The best option I had after talking with the Chrysler dealership was to to drive home praying my family didn't get stuck again.

Car then refused to move either further back to complete the street entry maneuver or forward to re-enter the driveway. At that point car was first pulled fully forward into driveway, then successfully backed out into the street and driven off.

On 2/24/2022, we were merging onto the highway when the van powered itself off. The engine light came on. We pulled over, got the van back on, and pulled back onto highway. Almost immediately, the van shut itself off again. On 2/25/2022, on the way to the dealership, the van once again powered itself off on the highway. After arrival to the dealership, the technician said he found a loose cable connecting to the transmission and apparently fixed it. On 3/2/2022, the van shut itself off yet again in the middle of an intersection. In each of these cases, we have been risking get hit by other vehicles. Had we not been able ?to pull off the highway in time, we could have been hit full speed while our van was completely shut off in the middle of the highway. When the van shuts itself off, it takes a while to be able to power it back on. There is no warning to when it will shut off.

The car stalled while driving. I was driving down a city street and the engine cut off and a message on the dash read "Put Vehicle in P to start." I pulled over and it took several minutes to get the car to restart. This happened several times on the way back to the dealership (we had only just bought the car the day before). Our safety and that of others on the road was put at risk because we were stopped in the middle of traffic and were unable to move. Pulling over across traffic once the car had stalled was also dangerous because I could not propel the car forward. I did manage to steer the car to the side of the road. After this happened several times in the course of a few blocks, I called for roadside assistance to tow the car back to the dealership. After the incident happened, the "check engine" light came on. The car is currently at the dealership awaiting repair.
