Four separate incidents but related incidents. Primary concern: vehicle turns off and puts itself in park without notice and cannot be restarted. Two incidents were in traffic at a stop light, and the vehicle could not be started for 12-15 minutes. Two times it occurred while the vehicle had been parked for a short period of time but could not be restarted. March 4, 2026 driver was stopped at a light, after about 20 seconds, driver received a dash message “Press brake and push button to start” indicating the vehicle was off, and the vehicle was in park. Driver depressed the brake and the vehicle started. March 7, 2026 driver pulled into a store and when they excited the vehicle, they notice their carplay was still attached to their phone/music. Driver disconnected their phone but notice a dash message “auxiliary connected.” Driver attempted to start the vehicle, but it would not start, however all of the lights, carplay, messages were cycling as if the vehicle was on. When driver attempted to start the vehicle there was no give to the brake and the vehicle would not start. Driver was able to start the vehicle 15 minutes later. March 27, 2026: Driver was stopped at a light and the vehicle turned off and put itself in park. Driver had to contact police t0 block traffic to prevent vehicle from being hit. Vehicle started after 15 minutes. Driver immediately took the vehicle to the dealership. April 2, 2026: Dealership service technician called and said they could not recreate the problem, that all codes were checking out, however they did see a intermittent short in the crank sensor and it was replaced. April 3, Owner pick up the vehicle at 5:30, drove five miles away. Put the car in park to get fuel and the vehicle would not start. Again – “press brake and push button to start” was indicated on the dash but the vehicle could not be started. There is no warning to when this happens. No sputtering, engine noise, just unusual turns off and goes into park.

Entered vehicle through driver’s side door. Put car in reverse and backed out of parking space. As the driver put the car into drive, we advanced approximately 25 yards, the rear tailgate opened and lifted without alerting the driver. No warnings, alarms or dash board notifications. Since we were in a parking garage, as the door lifted, it hit a concrete ceiling support, smashing the tailgate window and broke the tailgate lamps.

I scheduled an appointment with United Chrysler Jeep Dodge (111 85th St Brooklyn, NY 11220) for 2/2/26 and was told at scheduling the recall 06C/NHTSA 25V-302 and an oil change could be completed same day. This dealership charges a $150 per day storage fee for any vehicle that stays at the dealership and I wanted to avoid this fee and pick up the vehicle on the same day the service was completed. On arrival to the dealership, I was informed the part was not in stock and would need to be ordered. The part would be unavailable until 2/4 or 2/5. I asked why the part was not ordered on scheduling and I was told this was against policy. I asked to schedule the next appointment and I was told I could not until the part arrived. The manager then said all this information was in the letter I received and it is not the schedulers job to tell the customer if the repair can be completed upon scheduling. I left the car there to have the oil change. I was then called by the manager and told they will not touch the car and we can find another dealership. I confirmed they are refusing to do the recall repair as issued by FCA US and they confirmed yes. The next closest dealership is in the next state.

Van is only 2 years old, maybe 30K miles on it, and it dies while in motion from "electrical issues" of some sort, that are not even battery related. Never seen this in a vehicle ever. Happened to my wife twice in one week while transporting children. They are very lucky to be alive still. This model should not be allowed on the market in this condition.

Vehicle went into limp mode three times while driving on the highway, forcing us to pull over to the shoulder. Luckily there was a safe place to pull over but created a major safety concern. It triggered code P0A43-00. The dealer initially couldn't recreate the code on 12/18/25 at which point they cleared the code and returned the car to us. It happened again on 12/30/25 at which point the dealer followed Chrysler instructions to conduct pin drag test but no problem was found. Dealer drove 60 mile test and didn't recreate the fault and returned car to us. The same issue again occurred on 1/9/26 at which point Chrysler authorized invasive transmission repairs. This issue directly aligns with recall 23V-010 and NHTSA ODI investigation RQ24004

While driving the car an alert pops up with a red steering wheel and an exclamation point. The console states "Power Steering Unavailable Service Required". I brought the vehicle to a dealer and they stated they thought it was from low voltage due to a bad battery. They returned the car and the issue happened again. Oddly the you get the symptom first. Then you see an alert saying "LaneSense Off" then a ding where the console says "Power Steering Unavailable Service Required". So for a few seconds you might think lane sense off is the problem even though the reality is you have no more power steering. My family's life was at risk. Without power steering the vehicle was incredibly hard to control. It is very heavy and I had to use my full force to turn it, if it was my wife or someone less muscular they would have crashed. It has been inspected by the dealer and they returned it after replacing the battery and the issue re-occurred. I noticed this was an issue on a previous year model but it occuring on ours as well.
VEHICLE SPEED CONTROL,ENGINEJanuary 5, 2026
While driving at approximately 65 mph on I-35 in moderate traffic, the vehicle experienced a sudden and complete loss of motive power. I was positioned in the far-left passing lane at the time of the incident. A notification message appeared on the dashboard screen (likely the "Stop Safely" or "Service Hybrid System" warning); however, there was no accompanying audible warning tone to alert me to the severity of the issue before power was lost. The failure rendered the vehicle unable to accelerate, creating a severe safety hazard. I was forced to navigate from the far-left lane across multiple lanes of traffic to reach the exit while the vehicle was effectively crippled, moving at speeds under 10 mph amidst highway-speed traffic. I was eventually able to exit the freeway at this reduced speed. This was the second time this issue had occurred. The previous check engine light had cleared on its own and I assumed the vehicle was safe to drive.

This complaint concerns hardship caused by dealer scheduling and parts availability issues during a mandatory recall, not a defect in the recalled part itself. My vehicle was scheduled for a recall repair at an authorized dealership. The dealership confirmed the appointment but did not have the required parts available. I was informed of this only after dropping off the vehicle. As a direct result of this failure in scheduling and coordination: –I was required to rent a vehicle unnecessarily. –No loaner vehicle was available, despite recall instructions advising customers to ask dealers about alternate transportation. –I incurred rental charges, mandatory fees, fuel costs, and extra travel time that would not have occurred absent the dealer’s error. –I also experienced additional personal hardship, including having to transfer a child car seat between vehicles and adjust family obligations, such as waiting at a medical appointment, due to the unexpected need to return and manage the rental vehicle. The manufacturer’s customer care has limited reimbursement strictly to a standard policy cap and has declined to consider dealer-caused hardship or goodwill review, despite conflicting written guidance regarding reimbursement limits. This complaint is about whether recall compliance processes adequately protect consumers from avoidable hardship caused by dealer scheduling failures during mandatory safety recall repairs.

While driving at approximately 65 mph on I-35 in moderate traffic, my vehicle experienced a sudden and complete loss of motive power. I was positioned in the far-left passing lane at the time of the incident. A vague notification message appeared on the dashboard screen; however, there was no accompanying audible warning tone to alert me to the severity of the issue before power was lost. The failure was identified as a malfunction in the hybrid drive motor system, specifically triggering Diagnostic Trouble Code P0A43 (Drive Motor "A" Position Sensor Circuit Intermittent). This sensor circuit failure caused the high-voltage system to shut down unexpectedly. This created a severe safety hazard, as the vehicle was rendered unable to accelerate. I was forced to navigate from the far-left lane across multiple lanes of traffic to reach the exit while the vehicle was effectively crippled, moving at speeds under 10 mph amidst highway-speed traffic. The problem has been confirmed via the retrieval of the P0A43 code. There were no warning lamps, messages, or drivability symptoms prior to the immediate failure; the visual notification appeared simultaneously with the loss of power.

The front driver's side tire on my 2023 Chrysler Pacifica (77600 miles as of 12/1/25) has started making a loud rumbling sound regardless of my speed. This is happening when I turn the wheel from left to right and occurring every time I am driving. There are no warning lights on the dashboard. I had a mechanic evaluate the tire while on racks in the air and was shown the tire visibly rapidly bouncing and vibrating as the wheel was turned left to right. I am terrified to drive my van alone or with my family as the rumbling sound is heard loudly and consistently inside the van. This is my only mode of transportation and I am gravely concerned the wheel may indeed fly off the car and as a precaution I am driving slower than the speed limit on roads causing other drivers to become frustrated and close to hitting the back of my van. I need this issue addressed immediately to ensure the safety of everyone.

On September 5, 2025 I took my vehicle to be serviced at Earnhardt Dodge in Gilbert AZ. I informed the service attendant that I believed there might be a recall on my air bags. Upon checkout I didn’t see if the recall was serviced among other things and it wasn’t listed as being completed. On [XXX] a deer struck the front of my vehicle while driving at high speeds. The air bags didn’t deploy and the air bag light illuminated after the accident. In October 1st my vehicle was taken to Earnhardt Collison and a diagnostic check was done on my vehicle yet the light or anything else regarding the airbags was not listed in the appraisal after I notified Earnhardt and USAA that the airbag light was on after the accident. After they continued to ignore my request I demanded my car be transfered to a new body shop and it not be touched on October 3rd. On October 7th USAA finally agreed to transfer my vehicle to a new collision center but without my knowledge they conducted another appraisal which listed another pre and post diagnostics on the appraisal. When I brought this to the attention of my USAA adjuster he didn’t reply to my questions directly and instead gave a off the topic answer. The vehicle currently sits at a different body shop after approving a teardown and waiting on a new official estimate. I specifically requested any air bag light or diagnostic be noted. There is a recall with NHTSA Recall # 25V-302 with no known fix. This recall, which is a problem with the curtain air bags is a major safety issue. From the recall information: "Curtain airbags which do not have proper pressure retention may reduce occupant protection and increase the risk of partial and complete ejection and injury of an occupant through a side window during certain types of crashes." I drive my children in this car, and when I first learned of this recall, I asked Chrysler to provide me a replacement vehicle that was safe for my children. They refused to do this. They escalated my case to a case manager, who has not responded to my emails nor my phone calls since September 30th, 2025. The vehicle was inspected by the dealer in August, and the only issue with the car was the recall.

See attached document for complaint. I am writing to you with deep concern to report a serious matter regarding the safety of a vehicle I recently purchased and to respectfully request your intervention in a case that endangers not only my safety as a consumer but also that of the passengers transported in this automobile. On XXXX X, XXXX, I purchased a 2023 Chrysler Pacifica Wagon 40 Touring L 3.GL VG, VIN number [XXX] , through Napleton Chrysler Jeep Dodge Ram, located at 1460 E Osceola Pkwy, Kissimmee, FL 34744. The vehicle was financed by XXXXXXXX XXXXXXX LLC under loan number XXXXXXX-XXXXXXX. Prior to the purchase, the sales representative informed me that the vehicle had a recall. However, I was assured that it was a minor issue of no real consequence and that I could take it to any Chrysler dealership the following day for immediate resolution. Unfortunately, this statement turned out to be entirely false. To my shock, when I went to the dealership to address the recall, I was informed that the issue was related to the rear seat airbags and that no solution is currently available from the manufacturer. Since then, I have visited several Chrysler dealerships seeking assistance, and in every case, I was told that nothing could be done until Chrysler provides an official remedy. As a provisional measure, I requested a replacement or loaner vehicle to ensure the safety of the occupants-particularly those seated in the rear seats-but I was instructed to contact Chrysler's customer service directly. On August 25, 2025, I contacted Chrysler's customer service and spoke with a representative named XXXXX, who documented my case in detail. During that call, I also reported that the vehicle is experiencing additional operational issues, specifically with the engine and steering. On several occasions, the steering has locked up, preventing the wheel from turning and creating an even greater risk of an accident. I firmly believe that this vehicle should never have been sold or delivered in such condition, as it poses serious and unacceptable risks to occupant safety. The absence of functional rear-seat airbags represents a direct and potentially life-threatening hazard in the event of a collision. In my view, this constitutes a violation of the minimum safety standards that any new vehicle sold in the United States must meet. For these reasons, I respectfully request that the NHTSA intervene to investigate this case and assist me in obtaining a clear resolution from either the dealership or the manufacturer. At present, I find myself in a situation of insecurity, frustration, and financial detriment, compounded by the mechanical failures described and the lack of adequate support. I sincerely thank you in advance for your time and attention. I remain available to provide any documentation or additional information you may require to proceed with this formal complaint. Stellantis "released" recall for 2022-2025 Chrysler Pacifica vehicles. This specific VIN number involves recall 54C. I work for a dealership, Stellantis has a large order restrictions on the part(s) despite having over 6k parts available in parts distribution centers ready to be shipped in the US. The parts are currently limited to dealers only being able to order 2 parts every 7 days. On top of that, the recall has been "released" but inside the recall document, it is stating that only model years 2024-2025 are available, and have not actually released it for model years 2022-2023, despite the repair procedure (and parts) being the same for all model years. They will not pay any claims to dealers on this recall for 2022-2023 vehicles. This behavior is not isolated to this recall, and happens in numerous recalls released by Stellantis. For a safety recall, there should not be an order restriction to dealerships, especially if there is recall parts available in PDC's. If needed, I can provide screenshots of order restriction, and the current part supply in PDC's within the US.

On August 2, while traveling about 75 MPH, the check engine light came on along with the warning "Stop Safely – Vehicle Will Shut Off Soon". The vehicle started immediately losing power. We were driving in the fast lane in traffic so pulling over was difficult and dangerous. After we were to the side of the road, we turned the van off. After 10 minutes or so, we continued on and pulled over at a rest stop. We have an OBD2 and checked the codes. At first, there were no codes even though the check engine light was on. After checking a few times, there were 2 codes. They were P1C65 PD and P0A43. I took the van into the dealership on August 4. They saw one of the codes saved. On August 7 the dealership preformed a software update. They were not able to reproduce the issue. Today, September 19, while driving, the check engine light came on with the warning "Stop Safely – Vehicle Will Shut Off Soon". The vehicle again started immediately losing power. I recently purchased a 2023 chrysler pacifica hybrid that did not have a recall listed for the loss of power incident. Within 24 hours of buying the car it died on me while driving. I was driving on a two lane busy road when without warning the vehicle beeped loudly and suddenly I lost all ability to use propulsion. The car illuminated that I should brake and it will shut off soon but soon was immediate and I had zero time to safely exit the road. After dying a little turtle symbol showed up on the dash and the check engine light became illuminated. Once I was able to get the vehicle restarted the check engine light remained illuminated and ai arranged for the car to be seen at the dealer right away. I took it to the dealer within a few minutes of this event and they did the update that was supposed to fix something. Within 3 days it repeated the same event. It is now sitting at the dealership to be looked at again. I had no time to stop safely, had I been on the highway the ability to remove myself safely from the road would have been zero percent chance. How long before the government steps in and holds Chrysler accountable for their poor quality and repairs that fix nothing? I lost power completely. I lost the ability to maintain speed in order to safely remove myself from the road. My family could have been killed. These are family vehicles, out yourself in the shoes of someone with children, would you feel safe putting your kids in a car that could die on you at any moment without any real warning signs?

On 8/12/2025, he vehicle displayed a warning that said "Stop Safely, Vehicle will shut off soon" shortly after it was parked. It also displayed a red turtle and the check engine light. There was no warning or check engine light prior to the message that the vehicle was going to shut off. Fortunately, we had already parked safely, but were on the freeway traveling and about an hour away from home. It was 100 degrees outside, and we were stranded with no help from Chrysler customer care other than to tow the vehicle, leaving our kids and dog in a parking lot with no car. We declined this option and had it checked by a diagnostic computer at Jiffy Lube, which returned no diagnostic code. Eventually, all of the warnings and check engine lights went off, and we were able to take it home. We took it to the dealership at the earliest opportunity on 08/14, and they confirmed that this is a known issue and would require a transmission replacement. There is no timeline on the repair or replacement, only that it will take "a while". They do not advise that we take the van back because the issue could come nback at any moment and the vehicle could shut off without warning.

Sometimes the same problem occurs again after driving for several hundred miles per hour, and sometimes even after driving for tens of miles per hour. When this problem occurs, the vehicle cannot continue to drive and can only rely on inertia to slide. It must be parked on the side of the road, the engine must be turned off for one minute, and the vehicle can start again to drive normally. June 2025: Dealership’s First Attempt to Fix The dealership showed stored error codes P0A43 Drive Mode Code A Position Sensor Intermittent. But, the tech said they were unable to replicate the error. We were going uphill and unable to fully accelerate on the freeway, again putting our entire family in danger. We were luckily able to pull off of the freeway, restart the car several times, and continue driving–wondering the entire time whether the vehicle would again lose power, and sure enough it did. [XXX]: Third breakdown on freeway Later that same day, on our way home, the Red Turtle alert appeared again, the vehicle was unable to fully accelerate, and the message warned us that the car was about to shut itself down. This time, we were on the freeway with no exit available to us. We had to pull over onto an unprotected shoulder, with cars speeding by us on both sides. It would have been unsafe for any of us–but especially my toddlers–to exit the vehicle. This is a PHEV and upon returning to my vehicle after being plugged into a Rivian public EV charge station I found that the charger handle overheated and smoking. When I left the vehicle 45 minutes earlier it was charging with no issues whatsoever. When I removed that handle, a large piece of the charge terminal came out of the handle a remained attached to my charge port. It was one of the connecting sleeves that melted out of the charger and somehow arc welded itself to the male end of my charge port. I still cannot remove it and so now I am unable to charge my vehicle. I notified the property officials and also notified Rivian, but neither is claiming any responsibility to the damage to my charge port. I took my car my local Chrysler dealer and explained what happened but they denied any fault as well. My vehicle has less than 13k miles and still covered by new car warranty so I thought they might know why that happened. In my judgment what had happened is the charge cable itself had a weak wire or a bad, loose connection to the copper terminal inside the handle that caused high resistance and heat to build, then broke away and bound itself to my charge port once I removed the charge handle. When I try to followup with Rivian, best I can do is communicate with their front line phone customer service (they never allow me to speak with anyone authorized to make decisions about the matter), however Rivian did fix their public charger after the incident, but they're seem uninterested in assisting a fix to my vehicle, which I feel they are at fault So have claimed Chrysler had had charge port issues but they are denying assistance. I have an estimate of around $980.00 to repair my charge port, but neither Rivian nor Chrysler warranty are willing to make repair. I feel victim here, used equipment as advertised but being denied product liability claims by both parties, Rivian and Chrysler.

Took the car in on 5/10 which was the earliest appt. Said they did a software update, gave the car back. Took the car again on [XXX] when the same warning appeared again on the interstate, same situation disabled vehicle at high speeds on the interstate and no power. Said they changed a module related to the error and gave me the car back. Yesterday as I was driving on [XXX] 70 mph the car gave the same warning and shut off again, immediately I had no power, 30 mph. The dealer acknowledged my car is not the only one in his shop with this issue. They had my vehicle for nearly a month the time before. This vehicle is not safe to drive on the highways as it can shut off at any time. It's a known problem by Chrysler and their idea of fixing it, is issuing a warning which gives you almost no time to get off the highway before your speed is greatly reduced and even then you're stuck on the side of the highway with cars speeding by in an area with no shoulder or construction zone you'd be stuck in the middle of the road. Someone will wind up dead on the highway, I am lucky that in the 3 times this has happened I have not been hit and have been able to safely get to the shoulder, but I don't see Chrysler being able to fix this issue, the dealership did reach out to the manufacturer each time. I know my car is not the only one that has this issue and I'm concerned for the safety of anyone who owns one of these. A simple google search of this vehicle will show the problem is wide spread. On June 3, 2025, my 2023 Chrysler Pacific Hybrid gave me a warning message that it was about to shut down. This included a "red turtle" icon that I later learned is called "limp home mode." I was traveling on the interstate at highway speed at the time. I was able to pull over to the shoulder and turn the vehicle off. The risk of a car shutting down on the highway is a huge a safety risk to myself and other vehicles. Stopping on the shoulder of a highway, particularly given that it was early morning and not yet fully light out also felt very risky with other cars passing by. After turning the car back on, the limp home mode turned off, but the engine warning light was still on. I was able to re-enter traffic, which was also a safety issue having to deal with. When the dealership opened, I contacted them. They were familiar with the problem and were able to state over the phone that it was likely a transmission issue. After inspecting it that afternoon, they determined it would need a transmission rebuild. As of June 27, 2025, they still have the vehicle and have not yet made the repairs.

While exiting an interstate, the van felt like it had popped out of gear and lost power, and the instrument cluster had the message "Stop safely, vehicle will shut off soon." I was in the middle of three lanes on the ramp, and due to traffic and no shoulder I was unable to stop off the side of the road. The van died at the top of the ramp at the traffic light, causing a risk of being rear-ended by approaching traffic. I had my two children in the van at the time. After attempting to cycle the ignition several times and stepping out to wave traffic around as the traffic light cycled several times (causing a traffic backup), the van started and drove normally albeit with the Check Engine Light on. I have not had the time to take the van to a dealership for diagnoses, but looking online this is not an unknown problem and is related to the Pacifica Hybrid's transmission. Just wanting to make NHTSA aware of another event of this fault occurring.

After driving 15miles home from work, stopped at a traffic light to turn left. When attempting to proceed forward van stopped with an error code that indicated "Stop Safely Vehicle Will Shut Off Soon". The van shut down, and could not move even though it was blocking the intersection. Law enforcement had to be called as it was a safety hazard, and a tow truck had to be dispatched to remove the van from the intersection and tow to dealership for warranty service. The dealer provided warranty service as per Chrysler direction, which was a software update and reseating electrical connections. However, as per the notes from the dealer, "possible a loose connection was issue but still could be internal to power inverter module." As the repair was not a definite resolution for the issue, I contact Chrysler corporate to request a more definite repair. Their stance was that they considered the car repaired under warranty, and that they would not authorize any further work. Chrysler stated that they will not do anymore work until the issue happens again. The issue is that the condition that results is that the van shuts down - and it can happen anywhere, the middle of an intersection or on the freeway at highway speeds. Basically Chrysler has told me that even though they are not sure that the fix applied may not have remedied the issue that caused the van to shut off, they will not do what is required to ensure that it does not happen again. Chrysler has put the safety of my family second to their bottom line - by refusing to authorize the expensive work that it would take to remedy the problem, and ensure that my faily does not face a dangerous situation when driving our van.

I rented this car from Budget on Saturday, May 31, 2025. When I drove it on Sunday, June 1, 2025. The check engine light came on and the car gave a warning that it would shut off. The first time I saw this notification I was stopped at a stopped sign at a 4 way intersection. The car proceeded to shut off after some time (maybe 20 seconds or so, not sure). I was at a full stop and the car was in "Drive". When the car shut off it went into "Park" automatically. I turned the car back on and put it back in "Drive". The car would not move. I check to make sure the parking brake was not turned on and it was not. I put the car in "Park" and back into "Drive" and the car still did not move. I turned the car off a few more times before it switched back into gear normally. I was unable to return the car at that time. About an hour later as I was driving on the interstate in the left lane, the same warning light came on. I had to immediately move from the left lane of a 6 lane interstate all the way to the right shoulder. There was no exit or left shoulder. Once I got there the car idled for a moment and shut off; I am uncertain how long it was before it shut off. I waited a couple of minutes since I was also shaken up at having to move so many lanes on a busy interstate with so many lanes. I got to my destination after about 30 minutes. On the way home about an hour in to the drive, the same thing occurred. I moved onto to the right shoulder of a two lane interstate at this time. This time I shut the car off myself and waited a couple of minutes to turn the car back on. I then drove another 45 minutes home. Then this morning, same warning in my driveway before I left to return it. There were issues with the blind spot warning not working some of the time as well as the display screen would flicker on and off or just become disabled but still visible. I have reported this to the rental company but I want that car off the road.

Letter sent to AVIS after the incident: The image included is an example and not associated with incident Concern Regarding Recalled Rental Vehicle - Avis Rental Agreement [XXX] Dear Mr. Tucker, I am writing to share a concerning experience my wife and I had with an Avis rental. We rented a Chevrolet Pacifica Hybrid for a drive from Atlanta to Chicago. About an hour into our trip, the vehicle displayed a warning message: "Stop Safely. Vehicle will shut down soon." Following the instructions, we immediately pulled over, activated our hazard lights, and called Avis emergency roadside service while consulting the owner's manual. We then decided to move to a nearby gas station parking lot for safety. While still on hold, we researched the warning online (see link) and discovered it was a known recall for our vehicle. The recommended temporary fix—turning off the car, opening and closing the driver’s door, and restarting the engine—allowed us to continue. However, the warning appeared three more times during our trip, requiring us to stop and repeat the process. Though we managed the situation, it caused unnecessary stress and could pose a serious risk for other customers. After 15 minutes on hold, we disconnected from emergency road service as we no longer needed assistance. We called back later to inquire about switching vehicles, but chose to continue rather than delay our trip even further. This raises important questions: Why does Avis place vehicles with outstanding recalls into its rental fleet? What is Avis’s general approach to managing manufacturer recalls? If such vehicles are rented, why not inform customers in advance and provide a workaround? While I have contacted Avis/Budget about customer service concerns in the past, this is a safety issue. I urge you to escalate this matter to your management team and clarify how Avis plans to prevent similar situations. I would appreciate a formal response outlining your approach to recall management. We took it a second dealership this time with an active code, but the code cleared itself upon restarting the car after leaving it there. This vehicle is not safe to drive since it randomly goes into limp mode with no power. This poses a very dangerous and unsafe situation that can endanger me, my family, and other drivers around me.

FORWARD COLLISION AVOIDANCEJanuary 22, 2025
The contact owns a 2023 Chrysler Pacifica. The contact stated while driving approximately 25 MPH while approaching a vehicle that had stopped suddenly for a traffic signal, the Forward Collision-Avoidance brake failed to engage. The contact stated no warning light was illuminated. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the front collision system was working. The contact stated that the failure of the Forward Collision Avoidance braking system occurred again, and the vehicle was returned to the same dealer. The contact stated that the dealer had set up a soft barrier collision test and had driven the vehicle up to the barrier at 10 MPH and the Forward Collision Avoidance braking system failed to engage. The contact had taken the vehicle to two other dealers who stated that the front collision system was in operating condition. The vehicle was not repaired. The manufacturer was informed of the failure. The failure mileage was approximately 14,000.

This is the second time this happened in one week. Incident happened immediately after being sent home without repair by the dealership. Driving down the interstate, our car displayed the "Stop Safely. Vehicle will shut off soon." Red turtle light came on, and car lost all acceleration. We managed to pull over without being hit. Turned the car off and then back on. Check engine light was now on, but the car would drive. Hybrid battery was charged at 66%. In both shutdown instances, dealership says it is unable to replicate the problem, and therefore will not look further into the problem.

We were driving on the interstate at approximately 70 mph when our car gave the warning, "Stop Soon. Vehicle will shut off." A turtle warning light also lit up. We pulled over to the side of the road and turned the car off. After a few minutes, we were able to turn the car back on and drive home, though the check engine light was then on. We took it to the dealer the next day, with the check engine light still on, and the dealer was unable to reproduce or diagnose the problem. They said there were so many warning codes that they could not determine what was the problem and that if they followed what the codes were telling them, they'd have to replace the entire wiring system of the car. They also said that one or more of the codes was related to the transmission. They drove the car extensively and couldn't reproduce the shutdown. They are currently still trying to diagnose the problem with the car at the dealership. There were no warning signs from the vehicle prior to the incident. The incident occurred right around the time when we would have depleted the all-electric charge and switched over to using gas.

Keys were inside car with only an infant inside and the doors were locked with no other key fob anywhere nearby. There must have been an issue with the car’s safety system to unlock the car in this situation. There was no 3 beeps that would unlock the car. Car was parked in Walmart parking lot in the heat. Remote start was on, key fob was in drivers seat. The last door to close was the drivers side rear sliding door. Wife called me from a shopper’s phone since hers was also locked in the car. I frantically drove with the other key fob a few miles to where they were to unlock. It was a traumatic experience for her and our son who was stuck inside the car. Thankfully I got there in time to unlock with the other key fob and everyone is okay.

We have had our 2023 Chrysler Pacifica Plug-In Hybrid since [XXX]. This was a factory ordered vehicle. There have been no indications that there is any issue with the vehicle. About a week ago while driving the vehicle a warning came on the speedometer screen that said "Pull over safely the vehicle is about to turn off". The check engine light also came on. Once the vehicle was pulled over to the side of the road and stopped the vehicle turned itself off. After waiting a few minutes the vehicle was able to be restarted and driven about 5 minutes home. The check engine light remained on. We did not drive the vehicle again until we were taking it to the dealership to be inspected. The check engine light was on the entire drive from our home to the dealership, which is about 10-15 minutes. The vehicle has now been at our local Chrysler dealership for 6 days. The technician at the dealership is telling us the code stored in the vehicle memory means it needs a new transmission. We are being told they need to replicate the issue for the warranty to cover a new transmission being put in. If they cannot replicate the issue, we are to take the vehicle back and continue to drive it until it happens again and then immediately have it towed to the dealership so they can receive warranty approval to replace the transmission. My research has revealed this is a known issue in Pacifica Hybrids regarding stalling due to a transmission sensor. I called Chrysler to clarify the warranty process and if there is a recall on my vehicle. The warranty process is correct and there is currently not a recall for this issue on our Pacifica Hybrid. We have less than 3K miles currently on this vehicle. We were traveiling to the mountains for a vacation when the check engine light illuminated. Stayed on for 2 days and reset itself. We were in the Smokey Mountains and a warning came up to "Stop Safely Vehicle will shut off soon. Within a few minutes, all power was lost, all lights were inoperational including the 4 way hazards. After just a few minutes, the van restarted and operated as if nothing happened. The check engine light stayed on long enough for a Chrysler dealer to read the codes. Esentially, a problem with the High Voltage System Interlock Circuit A, Drive Motor A Inverter Performance are the major codes. A slew of others all related to this issue. The hybrid mechanic stated it is the same issues that plague the 2017-2021 Pacificas hybrids. Losing all power is not acceptable, especially in the mountains with a vehicle less than 4 months since delivery.

The contact received notification of NHTSA Campaign Number: 24V436000 (Back Over Prevention) however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact stated that the back-over prevention camera displayed still images, with blackouts, or was inoperable. The failure had become more persistent and then became a constant failure. The contact had to restart the vehicle to regain functionality or correct the malfunction. While turning off and restarting the vehicle, the radio was randomly activated without driver input. The contact believed that the failure had caused additional failures with other control panel functions such as the radio, heating, and the A/C system failing to activate as needed. There were no warning lights illuminated. The dealer was not contacted. The manufacturer was not made aware of the failure. The failure mileage was 14,000. VIN tool confirms parts not available.

I was driving my vehicle in a local road at speed about 20MPH and suddenly got a warning saying "Stop safely, Vehicle will shut off soon". Then the gas pedal stopped working. It was on the middle of the road. I switched off my car and started it again a couple of times and that warning was gone but the engine light was on. I was able to drive the vehicle and so the next day I went to the dealer to find the issue. After 4 days the dealer said they couldn't replicate the issue and also the engine light was not on anymore and so they returned my vehicle without doing anything. After a week I was driving on a freeway with all my family members (6 of us), 75 MPH the same thing happened again. I immediately pulled over to the shoulder, since the gas pedal was not working. I switched off the car and switched on then I drove the vehicle. This happened at least 6 to 8 times on my 300-mile drive. It was really scary that the vehicle got a weird warning and the gas pedal stopped working in the middle of the road. At one place there was no shoulder, so I had to stop on the road itself. After coming back, I dropped my vehicle with the dealer to find the issue. It's been 2 weeks but they are saying they couldn't replicate the issue. I bought this as a brand-new vehicle on April 2023, and it's just 14 months old.

On the afternoon of May 26, 2024, I received a warning "Stop Safely - Vehicle Will Shut Off Soon". I pulled to the side of the road but was still in an active traffic lane in a traffic circle and cars had to stop and/or swerve to avoid hitting my car. If this had been at night my safety would have been even more greatly compromised. The engine promptly shut off and would not restart. I was towed to the nearest dealer which still has not diagnosed or repaired the car. This may be related to recall 23V-010 and the ongoing RQ24004, of which the dealer had no knowledge. I took delivery of the car, a 2023 Pacifica plug in hybrid, on March 25th and it has less than 4000 miles on it.

We would have to shift to park, turn off, remove foot from brake then wait 1-5 minutes to restart. Took the vehicle into Rochester hills Chrysler jeep dodge and after several days of troubleshooting they replaced the steering module and claimed it was faulty. 24 hours after picking up the vehicle the same issue happened again. Took the vehicle back in annd they showed 15 different error codes that came up. Left the vehicle there for service and after a week of troubleshooting they were finally able to recreate the issue. They claim it was the RF transmitter in the tailgate causing the issue. If you are experiencing the same issues I recommend asking about the RF transmitter and steering module.

Vehicle has an auto start/stop feature which turns off the engine when the brake is applied and the vehicle is at a full stop. When stopped at a traffic light with the engine auto stopped, driver lets foot off brake when light turns green, but engine fails to start. Vehicle remains stopped in an active traffic lane, or potentially in the middle of an intersection. A chime and a display message guides driver to shift to park and manually restart the car. Has happened at least five times in 6,900 miles we have owned the vehicle. Dealer service department could not reproduce or find any log of the event occurring. They told me it needs to happen more frequently so they can reproduce it. I find other reports of this problem online going back to at least 2018.

I have a 2023 Pacifica with around 3k miles. I purchased brand new. In the few months that I have owned the vehicle, it has turned off about 5 times while driving. It loses all power. Engine, power, etc completely dies. I need to restart the vehicle, wait for start-up, and then I can continue driving. The vehicle has currently been at Arrigo Sawgrass Chrysler since 07/13/2023. Arrigo continues to say "They can not duplicate the issue and it does not show on computers " I called Chrysler Corporate for escalation. They told me that they have no record of the vehicle being in a repair shop until 07/25/2023. The bottom line is, this is a real problem. I have a 4-year-old, 2-year-old, and 2-month-old plus myself and my wife in the vehicle. What if this repeats and the car turns off in an intersection with a semi coming or worse? I understand the computer is not always accurate and it's a new vehicle. I do not feel safe if they return the vehicle without correcting the issue and do not want my family to be the test dummies for an issue that can be prevented. Chrysler is impossible to intervene and the dealership is only doing what they can. However, they don't get paid if they cant find issues so it's a tough situation.Thank you in advance.

While driving I got "rear seat alert" followed by "Car Shutting down in 9 secs" message, it happened twice. The first time on Northern State Highway in NY. I was driving with my family, and the car unexpectedly shutdown happened. Nothing worked, I had no power, no steering, no brakes, all gone. Our luck was that I was already on the right lane and just had to let the car stop in the curbside. Because of the messages, we thought the rear door was opened or something. After a few minutes, after several tries, the engine started, and we exit the highway and parked on the local road to try to understand what happened. We decided to continue our trip on the service road. We end up on Ocenside, NY on a two lane, busy street and it happened again, but at this time I was not able to move the car to the curb, the car stop completely on the middle of the road, no steering, nothing. Because of the incoming traffic, our hazard lights alone were not enough to warn the coming traffic. We though that an accident was eminent, for which we called 911 for help. I could not get out of the car because of the speed of the incoming cars. After several attempts, the car turned on again, I parked on the curb and I called the AVIS Rental Roadside Assistance, because I was not going to put my family, including an 87 year old in jeopardy. After we got home from the long, stressful day. I searched internet and for my surprise, there were several posts (links) relating the same issue I had. Including recalls from Chrysler about a short circuit, which could even cause a fire. Chrysler issued a NO PARKING INSIDE note, which is hard to understand. I feel it is my obligation, as a family person, to report this incident, because this car should not be in the ROAD. Avis is currently RENTING this car. Furthermore, for my surprise, I saw a ad on NBC, 5pm yesterday, 07/26/2023. It is just a matter of searching on internet to see the DANGER that families are exposed.

The motorized doors have no apparent safety features and will allow them to fully open and close even when obstructed. The rear hatch takes my full 200lbs body weight to stop from fully opening. It will lift the garage door and there is no safety mechanism to stop it from damaging the car door or the garage door. When closing it will not stop untilI use all my strength for about 20 seconds when it finally gives up. The open/close button simply reverses the direction. If something is pinched in such a way that stopping the door is the only way to reduce injury, it cannot be done. The door will simply keep reversing direction every time you push the button.... repeatedly moving against what is in the space. The side doors have similar issues. The button and door handle do not release the most, it only reverses the direction. Other vehicles I owned had some way to disengage the motor to prevent further injury. The 2023 Pacifica needs to be fixed to prevent injuries due to there being no method of disengaging the door motors.

With driver seated in the driver's seat, a child entered the vehicle and pressed the "Stow 'n Go Assist" button, instead of the "door close" button. From the Stellantis website: "There is a button on the B-pillar that allows for easier operation of the Stow ‘n Go feature. By pushing the Stow ‘n Go Assist feature, the front seat will move forward automatically to allow the second-row seat to be stowed. Pressing the button again moves the front seat back to its starting position" The driver's seat, while occupied, still moved forward, pinning the driver between the seat and the steering wheel. Pressing the "Stow 'n Go Assist" button again to attempt to free the driver resulted in an error message on the instrument cluster that an obstruction was detected, so the seat could not be moved (not exact wording, unfortunately). The driver, once calmed from the initial shock of the event, was able to force her way out of the seat (it was not easy for her to move out of the seat in this position), and only then did the seat return to normal operation. Van is available for inspection, we have NOT attempted to reproduce the issue, the event was reported to the local dealership and they indicate they will check for any TSBs or anything of the sort once it is there. Nobody has actually inspected it, and there were no warnings or errors related to this prior to the event occurring.
