Mazda North America Operations (Mazda) is recalling certain 2005-2006 Mazda MPV vehicles sold, or ever registered, in the states of Alaska, Colorado, Connecticut, Idaho, Iowa, Maine, Massachusetts, Michigan, Minnesota, Montana, New Hampshire, New York, North Dakota, Oregon, Rhode Island, South Dakota, Utah, Vermont, Washington, Wisconsin, and Wyoming. On July 9, 2019 this recall was expanded to include certain 2005-2009 RX-8 vehicles that were previously recalled under 17V-011 but have not yet been repaired. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.
Consequence: An inflator explosion may result in metal fragments striking the vehicle occupants resulting in serious injury or death.
Remedy: Mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate inflator, free of charge. The recall began July 6, 2018 for MPV vehicles. The recall began July 9, 2019 for RX-8 vehicles. Owners may contact Mazda customer service at 1-800-222-5500, option 4. Mazda's number for this recall is 2618F. Note: With the addition of the RX-8 vehicles, this recall fully supersedes recall 17V-011.

Mazda is also recalling 2005-2006 MPV vehicles in Alabama, Arizona, Arkansas, California, Delaware, the District of Columbia, Florida, Georgia, Hawaii, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maryland, Mississippi, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Virginia, West Virginia, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. On July 9, 2019 this recall was expanded to include certain 2004-2011 RX-8 vehicles that were previously recalled under 16V-354 but have not yet been repaired. The contact owns a 2005 Mazda MPV. The contact stated while driving 45 MPH and depressing the brake pedal, the brake pedal was depressed to the floorboard and the vehicle failed to come to a complete stop. As a result of the failure, the contact crashed into a tree on the passenger side of the vehicle. The driver's side air bag deployed; however, the passenger’s side air bag did not deploy. No warning lights were illuminated. A Police report was filed, but no injuries were reported. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. After investigating the failure, the contact related the failure to NHTSA Campaign Number: 19V781000 (Air Bags). The contact stated that the VIN was included; however, the vehicle had previously been repaired by the dealer. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 251,000.
Takata recall. The contact owns a 2005 Mazda MPV. The contact received notification of NHTSA Campaign Number: 18V404000 (Air Bags) however, the nearest dealer location was nearly 5 hours away. The manufacturer was notified of the issue and attempted to send a mechanic from the dealer to the residence for the recall repair however, the dealer declined. The contact was then referred to a Subaru dealer for assistance; however, the manufacturer failed to send the part to the dealer. The contact had gone back and forth with the manufacturer and other dealers but the vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact had not experienced a failure. Parts distribution disconnect.

I WAS COMING HOME FROM WORK DOING THE SPEED LIMIT AND MY CAR WOULD NOT STOP I PRESS THE BRAKE ALL THE WAY TO THE FLOOR AND IT STILL DID NOT STOP I PULLED UP MY EMERGENCY BRAKE AND THAT DID NOT WORK SO FINALLY I GOT THE CAR TO STOP BECAUSE I KEPT PUMPING THE BRAKES AND THE CAR STARTED SLOWING DOWN AND I THREW IT IN PARK I WAS SO SCARED I SEARCHED AROUND FOR RECALLS ON THE BRAKE AND THE EMERGENCY BRAKE THE ONLY RECALL I SEEN WAS IN 2017 IN DETROIT. IN AUGUST OF 2016 RECEIVED A NOTICE FROM MAZDA STATING MAZDA DECIDED THAT A DEFECT WHICH RELATES TO MOTOR VEHICLE SAFETY EXISTS IN MAZDA 2005 MPV. THE RECALL NOTICE CONTINUES TO STATE DO NOT DRIVE THE VEHICLE BECAUSE HIGH LEVELS OF HUMIDITY CAN CAUSE THE PASSENGER FRONTAL AIR BAG TO RUPTURE AND COULD RESULT IN METAL FRAGMENTS STRIKING THE DRIVER OR OTHER VEHICLE OCCUPANTS RESULTING IN SERIOUS INJURY OR DEATH. ON 10-31-16, I CALLED OAK TREE MAZDA, SAN JOSE TO SCHEDULE THE APPOINTMENT. I PROVIDED THEM WITH MY VIN NUMBER AND WAS TOLD BY THE PARTS DEPARTMENT THEY DID NOT HAVE THE PART IN STOCK AND WOULD NEED TO ORDER IT. THE SAID AS SOON AS THE PART CAME IN THEY WOULD CALL ME TO SCHEDULE THE REPAIR. NEVER RECEIVED A CALL BACK WITHIN 1 MONTH. CALLED AGAIN AND WAS IN ESSENCE TOLD THE SAME INFORMATION TAT THEY WOULD CALL ME BACK WITH INFORMATION. PROVIDED THEM WITH MY CORRECT PHONE NUMBER AND AS OF THIS DATE, 2-9-17 HAVE NOT RECEIVED A CALL BACK TO SCHEDULE MY APPOINTMENT. PLEASE ASSIST ME IN GETTING THIS ISSUE FIXED SO WE CAN DRIVE OUR VEHICLE. I AM NOT SURE IF THIS INVOLVES THE "TAKATA RECALL" AS OUR PAPERWORK JUST INDICATES, FRONT AIR BAG INFLATOR.
TAKATA RECALL HAS AFFECTED MY VEHICLE AND AFTER 7 MONTHS THE COMPANY NOR THE LOCAL DEALERSHIP HAS DONE ANYTHING TO ADDRESS THE DEFECTIVE PART. WE HAVE BEEN NOTIFIED TO NOT ALLOW ANYONE TO RIDE IN THE PASSENGER SIDE OF THE VEHICLE AND THIS IS TERRIBLY OBSTRUCTIVE TO OUR TRANSPORTATION NEEDS. I BELIEVE FAR TOO MUCH TIME HAS ELAPSED AND STILL THE COMPANY NOR THE LOCAL DEALERSHIP HAS ADDRESSED THIS VERY SERIOUS ISSUE.

WHEN LOOKING AT MY VEHICLE, I NOTICED GREASE STREAKS DRIPPING FROM A LINE ON MY ALLOY WHEEL. AFTER CLEANING THE GREASE OFF, WE NOTICED A CRACK COMPLETELY THROUGH THE SPOKES ON MY RIM. THIS IS THE SECOND TIME THIS HAS OCCURRED TO MY VEHICLE SINCE WE BOUGHT IT BUT I WAS TOLD THERE IS NOT A PATTERN OF DEFECTS. I WAS TOLD ON BOTH OCCASIONS, AT MAZDA DEALER SERVICE DEPARTMENTS, MY VEHICLE WAS UNSAFE TO DRIVE. MAZDA USA TOLD ME NO PATTERN OF INCIDENTS HAVE OCCURRED AND ITS NOT A SAFETY ISSUE ON THEIR END ALTHOUGH THEIR SERVICE DEPARTMENTS SAY OTHERWISE.
